Demonstrating the Value of Enabling Services at Health Centers
Enabling Services are a hallmark of Community Health Centers, connecting vulnerable patients to supports that enhance their ability to become active partners in their own health care. Examples include transportation, interpretation or translation services for patients with limited English proficiency, referral for food or housing support, financial eligibility assistance for uninsured patients, outreach and more. This webinar will help participants think about how tracking and evaluating these services is essential for health centers to demonstrate their value to patients, providers, payers, policymakers, etc.
Learning Objectives:
- Explain the importance of Enabling Services data collection (ESDC) and documentation.
- Discuss the role of Enabling Services staff for value¬-based healthcare transformation and addressing Social Determinants of Health.
- Identify resources to launch or expand an ESDC initiative.
